National Specialty Survey
The PWDCA SEAT committee and the PWDCA Board are interested in your feedback on the National Specialty program.

This year's survey builds on the survey we conducted last year. You will note some questions are repeated from last year's survey as we felt it important to update our planning data based on influences of the current economic climate.

We are soliciting feedback from members and non-members who attend our specialties. If you, or someone you know, are unable to conduct the online survey please download the print version. The completed survey may be mailed to me at the address below and will be included in the final survey results.

  - Please share the availability of the test with others who are not on the lists.
  - Results may take as long as 4-6 weeks depending on the amount of samples received.
  - DO NOT call the University for results. It will only interrupt the researchers and slow things down. Test results will ONLY be available by email, fax or snail mail.
  - If you supply an email address on the forms, results will be emailed the same day that the certificate is mailed.
